offer valid for purchases made between 1/1/2022 and 12/31/2022. To receive your rebate via a prepaid reward card, all fields must be completed online or through a mail-in rebate form. The original, itemized veterinary invoice listing the products purchased, the purchase price and date of purchase, and the circled invoice number must accompany the submission. Refund request must be submitted on or before 1/31/2022. all doses must be of the same product and size, for the same pet, and purchased on the same invoice date to qualify for a refund. for value22 the veterinary invoice must reflect a purchase of at least two ( 2) or four (4) doses of bravecto® (fluralaner) and/or six (6) or twelve (12) doses of sentinel® spectrum® chewable or flavored®. limit of 10 rebate submissions or a maximum limit of $750 (10x $75) during the life of the offer per household, address or person and reserves the right to confirm identity. for bravo22 the veterinary invoice must reflect a purchase of at least two (2) or four (4) doses of bravo22® brand products (fluralaner). limit of 10 rebate shipments or a maximum limit of $250 during the term of the offer per household. for the sentinel22 veterinary bill it must reflect a purchase of at least six (6) or twelve (12) doses of sentinel® brand products. limit of 10 rebate shipments or a maximum limit of $250 during the term of the offer per household. only one offer can be redeemed per invoice and this offer cannot be combined with other offers. offer can only be redeemed by pet owner and must be 18 years or older to redeem. offer available on purchases from clinics in the fifty (50) united states or the district of columbia only. purchases from online pharmacies or other outlets not associated with your veterinary clinic are not eligible. fraudulent mailing could result in federal prosecution under the mail fraud statutes (title 18, sections 1341 and 1342). the use of multiple addresses to obtain additional rebates may constitute fraud and may result in prosecution. chewable sentinel® spectrum® (milbemycin oxime/lufenuron/praziquantel). dogs should be tested for heartworm prior to use. Mild hypersensitivity reactions have been observed in some dogs carrying high numbers of circulating microfilariae. treatment with less than 6 monthly doses after the last exposure to mosquitoes may not provide complete heartworm prevention.
